This evaluation scheme aims to describe the process of monitoring and evaluating the project “Sports Organizations Matching Social Inclusion Issues” (SmatchS), namely identifying the data that will be gathered as well as the methods and strategies that will be used to collect the information for evaluation. Moreover, it shows how the evaluation results can be used to improve the ongoing project activities and guarantee the sustainability of the measures implemented within the SmatchS project. This evaluation scheme serves not only as a static evaluation plan but also as a dynamic tool – a “living document” – that can be adapted at any time on an ongoing basis to reflect program changes and adapt information, if necessary.
